Daycare Subsidy Application Checklist

Use this template to gather paperwork, contact your CMSM/DSSAB, and keep your file active while you wait for funding.

1. Confirm your need for care

Collect proof of employment, full-time study, job search, or medical requirement for each guardian. Include shift letters that show early mornings, evenings, or weekend work if applicable.

2. Gather income documents

Download your latest Notice of Assessment (line 23600 net income) plus the last 8–12 weeks of pay stubs or invoicing history for self-employed workers.

3. Join the centralized waitlist

Register on your municipality's system (ex: Toronto Children’s Services, Peel, York). Rank several centres; DaycareLocator links directly to their profiles so you can copy license numbers into the application.

4. Submit forms

Upload documents through the CMSM portal or deliver them in person. Keep confirmation emails—many regions require proof if you need to appeal or update information.

5. Follow up monthly

Add a reminder to confirm your file is still “active.” If you change jobs or income, update the caseworker immediately to avoid losing your place on the waitlist.

Document checklist

  • Notices of Assessment for every guardian (most recent tax year)
  • Pay stubs or proof of self-employment income (8–12 weeks)
  • Employment letters or school timetables confirming hours
  • Proof of residency (lease, mortgage statement, utility bill)
  • Child's birth certificate, passport, or immigration document
  • Court orders, medical letters, or special-needs plans if applicable

Timeline expectations

Week 0–2: Submit your package, confirm receipt, and add your caseworker’s contact information to your phone.

Month 1–6: Check in monthly. Update any income changes and keep touring. Use our compare tool to track availability.

Month 6+: When offered a subsidy, you typically have five business days to accept. Have a preferred centre ready or you may need to defer.
